Multiple Choice (One Answer)
A multiple choice question requesting just one answer will appear with radio buttons next to each answer option.
Start by typing your question text.
If your question has many possible answers, you can decide if you want the response options to be listed in a single column or multiple columns.
Then, list your response options, typing each on a new line. You can check the box to require this question. You should try to limit the number of required questions in your survey to make it easy for respondents to give feedback however they feel comfortable.
If you check the “Other†box, your survey takers will be able to write in additional answers. You can customize the text that appears in the Other field.
Click Save & Close to insert your question.
You'll see your new survey question appear on the Survey Design page.
