If you choose the Textbox question type you’ll be able to provide a list of items with open text boxes for respondents.
Type your question in the Question Text Box.
Then type your answers in the Answer Options box, entering each option on a separate line. You can also check the box to make this question required.
Click Save & Close to insert your question.
You'll see your new survey question appear on the Survey Design page.
